I have flat feet. I've had them my entire life, and as a physical therapist who spends most of my day watching people move, I've learned to see my own feet the same way I see my patients' feet: as the foundation everything else is built on. A study on adults with flatfoot found that arch support insoles reduced fatigue in the lower leg muscles during uphill and downhill walking compared to flat inserts, and also lowered peak oxygen uptake during the same tasks (Huang et al., Journal of Healthcare Engineering, 2017). In plain terms, the right support lets people walk longer with less strain on the muscles doing the work of stabilizing a flatter arch. A separate study measuring plantar pressure and contact area found that arch support insoles improved how pressure was distributed across the foot and reduced stance time compared to flat insoles in people with flatfoot (Huang et al., PLOS ONE, 2020).

That's the honest case for flat feet insoles: better pressure distribution, less muscle fatigue, and improved stability during walking. For a lot of people, that translates into less end-of-day soreness in the feet, and sometimes further up the chain in the knees, hips, or lower back.

What Flat Feet Insoles Can't Do (And Why That Matters)

Here's where I want to be straight with you, because I think this industry oversells insoles constantly. Insoles help relieve symptoms while you're wearing them. They don't permanently reshape a fully developed arch.

A prospective study following young patients using medial arch support insoles over several years found real improvements in several structural measurements, but noted that those measurements remained outside the normal range even after long-term use, and that the untreated comparison group showed similar improvement over the same period (Choi et al., Foot and Ankle Surgery, 2020). That study looked at growing feet, where some natural change is still possible. In a fully mature adult foot, the expectation should be even more conservative. Insoles are a support tool, not a way to permanently reshape the arch.
